When we look at a lot of accidents, we think how could that little thing cause this accident? Well, this video demonstrates that really well. Instead of calling each one of those events a success, think of them as failures. Each item by itself is performing its function to the limit of its ability. In other words it has been optimized. If it's stressed by some un-expected event, such as the failure of another part of the system, it too may fail.

Safety is achieved by making each element of the system less unstable, or otherwise add sufficient margin, and placing some form of barrier between the elements, where it is practical. This is called Robust Design.
